Kino should calm down and find some alternative form of treatment. I, however, would pretend to be white and hope that works in getting treatment.
One [good] idea is to disguise, but if that does not work, for story purposes, I would say to break in as their last resort.
First, though, he should probably demand treatment and ask to pay in various forms, as anyone would do. However, that probably would not work very well, as the doctor might scoff and reply, "I do not want you to work for me. If I wanted to have an ugly pet to neglect, I would taken [sic] that scorpion that stung your pathetic baby so I would not have to deal with idiots believing I am a veterinary! Although, honestly, go live in the jungle!"
I think that offering yourself to someone is a very rational decision. However, the doctor says:
"Death: something we all try to avoid, but did you know that it can prevent the onset of illnesses? That is correct! With death, you do not have to deal with the stresses of life! So go ahead and try death today!"
